Minor criticism: the first thing I noticed was NOT the hand -- I didn't pick up on that until I saw your comment about it, and I still don't think it's that bad (fingers might be a bit long, but the character's face is so strong it draws my eye away from the hand anyway). The first thing I saw was that the woman's forehead seems to be way too exposed. It looks like her hairline begins a third of the way back on her skull -- if she were a man I'd say she's about 50 and losing her hair. ^.^ But based on hair color, gender, and the texture you used on the skin, I'd guess she's intended to be like 30yo maximum.

Or maybe it's a cult of people with really really high foreheads and the punk behind her is just Igor. In any case, on the whole I enjoy the image, but as far as flaws that's what my eye saw so I wanted to comment before you spend a lot of time worrying about the hand if you're planning to do further work on it at all.
